Advance Care Planning is a process of communication between you, your loved ones and care providers about your health care wishes. An Advance Directive is a legal document that explains to others the kind of medical care you want or don’t want. It is used when you are unable to communicate this information yourself. Advance directives must be prepared ahead of time, when you are able to make your own decision.
